Who We Are
Revive Social Services is a home and community-based service provider, specializing in serving diverse populations. As an enrolled Housing Stabilization Services provider, we are licensed to offer basic support, intervention, in-home support, and employment services. We serve individuals enrolled in HCBS waivers and the Elderly Waiver, and we also provide Social Security Advocacy for SSI applications.
Our work directly addresses health disparities by delivering culturally competent services to vulnerable populations who face multiple barriers to equitable care, including language obstacles, structural discrimination, and low income. We help individuals access essential programs to stabilize income, secure health care, and receive in-home support. Additionally, our housing stabilization services assist individuals and families in finding housing, applying for resources to cover moving costs, and connecting with other benefits and support systems. Each client receives personalized support through one-on-one case management, ensuring their unique needs are met and services are coordinated effectively.
